DATA PRIVACY CONSENT

Under R.A. 10173 of 2012

By providing the details requested by Baguio Country Club, by reading the Data Privacy Policy and by checking I agree, I hereby freely, willfully and voluntarily give my full consent to the collection, storage, processing, use, recording, organization, updating, modification, retrieval, consultation, consolidation, blocking, erasure and destruction (collectively referred to as “Use”) of Baguio Country Club Human Resource Office, its staff, for any or all sensitive, personal and privileged information I disclose to the Club for the purpose of:

A. Use (The Club uses personal information for the following purposes:)

• For pre-employment and interview purposes
• To conduct appropriate due diligence checks
• To perform other actions necessary or desirable in the implementation of your employment contact.
• Communicate relevant services, advisories including responses to your queries
• Comply with the requirements of the law and legal proceedings such as court orders; comply with legal obligation or to prevent imminent harm to the public security, safety or order.
• Process information for statistical, analytical and research purposes.

B. Collection (e.g. type of data collected, mode of collection, person collecting information, etc.)

• Personal Information such as: First name, Middle name, Last name, Extension name, Email address, Contact number, Place of birth, Current and Provincial addresses.
• Sensitive Personal Information such as: Age, Date of birth, Gender, Civil status, Valid ID's, Education profile, Record of service/employment, Government examination/s taken and Trainings/seminars attended.
• Privileged Information such as: Relatives working in Baguio Country Club.

C. Storage, Retention and Destruction (e.g. means of storage, security measures, form of information stored, retention period, disposal procedure, etc.)

We will retain documents containing your personal data:

• To the extent required by or pursuant to our contract;
• Only to the extent that we are required to do so by law;
• Destruction through database purging is done after five (5) years of retention
• Your personal information shall be disposed or discarded in a secure manner that would prevent further processing, unauthorized access, or disclosure to any other party or the public;

D. Disclosure and Sharing (e.g. individuals to whom personal data is shared, disclosure of policy and processes, outsourcing and subcontracting, etc.)

We may disclose your personal information to the following as reasonably necessary purposes

• Only to authorized Club personnel and departments such as emergency incidents and events

I hereby certify that all information provided are true and correct. I authorize the Club to verify all of this information. I recognize that I am entitled to certain rights in relation to the information, including the right to object to its use, and to access, correct and request deletion of the same. However, I accept that the Club cannot delete the information without restricting or removing its ability to effectively execute its responsibilities to facilitate transactions with government and regulatory bodies for purposes of complying with applicable and pertinent laws and other regulations for the above purposes indicated.

I further understand and agree that the Club may not accommodate a request to correct and delete the information if the Club believes the same would violate any legal requirement or cause the information or historical transactions to be incorrect.

I hereby release the Club, its Board of Directors, Officers, Management and personnel from any liability whatsoever, including but not limited to liability under the Data Privacy Act of 2012 for the use of the information for the foregoing purposes, and any liability in connection with or arising from any activity that may occur or result from there from.
